Authorization
I hereby grant permission to Eric Thrift ("the Researcher") to produce audiovisual recordings of me ("the Recordings") at the location(s) and on the date(s) indicated at the bottom of this form, within the context of the research project identified above.
I have been informed of the objectives, methodology, and expected outcomes of this project, and have consented to participate based on a full understanding of the potential risks and benefits.
Copyright
I retain the right to be identified as a contributor to and joint author of the Recordings for copyright and licensing purposes.
In any public distribution or presentation of the Recordings or derivatives I agree to be credited by my real name or pseudonym as indicated below. My use of a pseudonym in no way limits my rights to the Recordings under Mongolian and international copyright law.
Privacy and Access
I retain the right to review the Recordings and to request that any sensitive personal information be edited out. Unless I specifically authorize public access to the Recordings, no one may publish, broadcast, or otherwise disseminate the Records to third parties.

☐ I agree to have the Recordings stored permanently in a research archive, subject to the conditions below.

I am aware that my complete anonymity cannot be guaranteed if I allow access to the audiovisual records, as I may be identifiable to others based on visual evidence.
- ☐ I wish to be identified anonymously in the written transcripts and cataloguing records. A pseudonym will be assigned instead of my real name, and all references in the transcripts from which my identity could be directly known will be edited out.
- ☐ I agree to the archived Recordings and/or transcripts being made available to other participants in the current research project.
- ☐ I agree to the archived Recordings and/or transcripts being made available to other scholars (professional researchers and graduate students, who have made a written request for access to the Records for research purposes).
☐ I agree to the archived Recordings and/or transcripts being made openly accessible under the terms of the Creative Commons Attribution-NonCommercial-ShareAlike 3.0 license (attached).
Records licensed for public use may be freely copied, translated, quoted, used as part of a documentary film, or otherwise used by anyone for research, teaching, or similar non-commercial purposes.

Moral rights
I consent to the production and use of the Recordings on the understanding that my moral and privacy rights are separate from basic copyright terms, and are in no way limited by the licensing and use terms I have selected above. Specifically, my consent is based on the understanding that the Recordings will not be used in any way that:
- is prejudicial to my honour or reputation;
- exposes additional private information not revealed in the Recordings without my authorization; or
- suggests an association with, or endorsement of, a commercial product or service by me without my permission.
Release
I hereby release and hold harmless the Researcher and any employees or agents acting on his behalf from and against any claims, damages or liability arising from or related to the use of the Recordings, including but not limited to claims of defamation, invasion of privacy, or rights of publicity or copyright infringement.
I am 18 years of age or older and I am competent to contract in my own name. I have read this document before signing below, and I fully understand its contents, meaning and impact.
